I hadn't heard of that show before. I'll have to read more about it later. At first, I thought the clip was from a David Lynch film or something similar.

Last three drives in that game for both teams went 21 plays, 193 yards, 2 TDs (for one team), and a FG to win the game.

The previous three combined drives for both teams went 8 plays, 3 yards, 2 punts, and an interception. The winning team had 17 plays for 56 yards in the fourth quarter. The losing team (losing at the start of the fourth and game) had 20 plays, 162 yards (out of a total of 383 for the game), 2 TDs, 1 missed FG, and 1 interception.

i'm Dallas in the game vs. Denver

Denver ended up winning by 8, would've been by 1 but for some reason (i'm guessing hurry up rules) i went for it on 4th and 16 turning over the ball on downs on my own 24 yard line

the game up to the beginning of the 4th was:
Denver - 243 yards, 1 TD, 2 FGs
Dallas - 263 yards, 2 TDs, 1 FG, 1 Safety
Score at the start of the 4th: Denver 13, Dallas 25

4th Quarter alone:
Denver - 203 yards, 4 TDs
Dallas - 108 yards, 1 TD

and here's a mind-boggling play from earlier in the same drive that would've given me the win

and another big problem i had that i can't understand is my 88 accuracy QB was consistently missing receivers by several yards when he passed

One thing that I noticed is that fatigue rates for OL and DL don't seem to match the rest of the positions. OL and DL routinely play almost every snap that they can, but all of the skill positions - RB, WR, and CB - routinely sub. This was really apparent when I had turned the fatigue rates down to 25 for preseason and my OL and DL were leading the team in snaps while my star CBs were getting very few snaps per preseason game. Biomechanically, a player that weights 270+ pounds is going to fatigue faster than a player that weights 195 pounds. They are just more likely to play through the fatigue if you're an OL or even some DL. That's the reason why everyone talks about rotational DL and not rotational CBs.
